Addiction and Misuse

    The genuinely low addiction potential of classical psychedelics, how this differs for MDMA and ketamine, and what the research on psychedelic-assisted treatment for other addictions suggests.


    Classical psychedelics - psilocybin, LSD, mescaline, and DMT - have consistently low scores on addiction and dependence measures in the available research. This is not simply an ideological claim; it reflects measurable pharmacological properties. These substances do not activate the dopaminergic reward pathways associated with addictive drugs in the way that stimulants, opioids, alcohol, and nicotine do. They produce rapid tolerance, which makes frequent use pharmacologically unrewarding. And they tend not to produce compulsive craving in the way that substances with high addiction potential do.

    This picture is more complicated for MDMA and ketamine, two substances that share some properties with classical psychedelics but operate through different primary mechanisms. MDMA carries more significant neurotoxicity risk with frequent use and can produce dependence in some users. Ketamine is an NMDA receptor antagonist with dissociative properties that differs pharmacologically from classical psychedelics; it has recognized addiction potential, particularly when used frequently, and ketamine bladder syndrome is a serious physical risk of heavy chronic use.

    For ibogaine, another substance with psychedelic properties and significant clinical interest, the picture is again different: it is not considered addictive and has been studied specifically as a treatment for opioid dependence, but it carries serious cardiac risks that require medical screening.

    The low addiction potential of classical psychedelics does not mean that problematic relationships with them cannot develop. Some people use these substances repeatedly in ways that substitute for other forms of coping or processing, or use them as a means of ongoing avoidance rather than genuine engagement with what arises. This is not dependence in the pharmacological sense, but it can become a psychological pattern worth examining.

    The research finding that psychedelic-assisted therapy may actually reduce addictive behavior toward other substances (alcohol, tobacco, opioids) is one of the more intriguing dimensions of this field - a potential reversal of the ordinary relationship between drug use and addiction risk.
